Tuy nhi\u00ean, t\u1ef7 l\u1ec7 ph\u00e1t hi\u1ec7n s\u1edbm c\u00f2n h\u1ea1n ch\u1ebf: tr\u00ean 90% ng\u01b0\u1eddi m\u1eafc ch\u01b0a bi\u1ebft t\u00ecnh tr\u1ea1ng b\u1ec7nh; s\u1ed1 ca \u0111\u01b0\u1ee3c t\u1ea7m so\u00e1t m\u1ed7i n\u0103m ch\u1ec9 kho\u1ea3ng 8.000. [2] Nhi\u1ec1u b\u1ec7nh nh\u00e2n \u0111\u01b0\u1ee3c ch\u1ea9n \u0111o\u00e1n mu\u1ed9n \u1edf giai \u0111o\u1ea1n 3\u20135, k\u00e8m theo c\u00e1c bi\u1ebfn ch\u1ee9ng nh\u01b0 tim m\u1ea1ch, thi\u1ebfu m\u00e1u, r\u1ed1i lo\u1ea1n \u0111i\u1ec7n gi\u1ea3i\u2026l\u00e0m t\u0103ng nhu c\u1ea7u \u0111i\u1ec1u tr\u1ecb thay th\u1ebf th\u1eadn v\u00e0 nguy c\u01a1 t\u1eed vong. [2,3]<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u1ede giai \u0111o\u1ea1n cu\u1ed1i, g\u00e1nh n\u1eb7ng \u0111i\u1ec1u tr\u1ecb r\u1ea5t l\u1edbn. N\u0103m 2023, \u01b0\u1edbc t\u00ednh c\u00f3 kho\u1ea3ng 46.000 ng\u01b0\u1eddi b\u1ec7nh c\u1ea7n l\u1ecdc m\u00e1u trong b\u1ed1i c\u1ea3nh n\u0103ng l\u1ef1c h\u1ec7 th\u1ed1ng c\u00f2n h\u1ea1n ch\u1ebf (kho\u1ea3ng 7.000 m\u00e1y l\u1ecdc). T\u1ef7 l\u1ec7 t\u1eed vong sau 5 n\u0103m \u1edf nh\u00f3m ch\u1ea1y th\u1eadn kho\u1ea3ng 50% [4]. Chi ph\u00ed ch\u1ea1y th\u1eadn nh\u00e2n t\u1ea1o trung b\u00ecnh 9,6\u201322,5 tri\u1ec7u \u0111\u1ed3ng\/th\u00e1ng (t\u01b0\u01a1ng \u0111\u01b0\u01a1ng 6\u20137 l\u1ea7n l\u01b0\u01a1ng t\u1ed1i thi\u1ec3u); ph\u1ea7n t\u1ef1 chi tr\u1ea3 c\u00f3 th\u1ec3 l\u00ean t\u1edbi 80% thu nh\u1eadp h\u1ed9 gia \u0111\u00ecnh c\u1eadn ngh\u00e8o. [5-8], Chi ph\u00ed cho 1 ca BTM l\u1edbn h\u01a1n thu nh\u1eadp b\u00ecnh qu\u00e2n \u1edf Vi\u1ec7t Nam n\u0103m 2018.<\/span><\/p>\n<p><b>IMPACT-CKD: g\u00f3c nh\u00ecn m\u1edbi v\u1ec1 ph\u00e1t th\u1ea3i v\u00e0 chi ph\u00ed, \u0111\u1ecbnh h\u01b0\u1edbng m\u00f4 h\u00ecnh ch\u0103m s\u00f3c b\u1ec1n v\u1eefng<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Kh\u00e1c v\u1edbi c\u00e1c b\u00e1o c\u00e1o v\u1ec1 d\u1ecbch t\u1ec5 v\u00e0 l\u00e2m s\u00e0ng,\u00a0IMPACT-CKD mang t\u1edbi g\u00f3c nh\u00ecn m\u1edbi v\u1ec1 ph\u00e1t th\u1ea3i carbon v\u00e0 chi ph\u00ed y t\u1ebf\u00a0trong qu\u1ea3n l\u00fd b\u1ec7nh th\u1eadn m\u1ea1n (CKD) t\u1ea1i Vi\u1ec7t Nam. K\u1ebft qu\u1ea3 cho th\u1ea5y ba ngu\u1ed3n ph\u00e1t th\u1ea3i l\u1edbn nh\u1ea5t \u0111\u1ebfn t\u1eeb\u00a0nh\u1eadp vi\u1ec7n,\u00a0di chuy\u1ec3n b\u1ec7nh nh\u00e2n, v\u00e0 \u0111\u1eb7c bi\u1ec7t l\u00e0\u00a0\u0111i\u1ec1u tr\u1ecb thay th\u1ebf th\u1eadn\u00a0(l\u1ecdc m\u00e1u, gh\u00e9p th\u1eadn&#8230;) \u1edf giai \u0111o\u1ea1n 5.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ph\u00e1t th\u1ea3i c\u00e1c bon t\u1eeb ho\u1ea1t \u0111\u1ed9ng ch\u0103m s\u00f3c m\u1ed7i b\u1ec7nh nh\u00e2n CKD giai \u0111o\u1ea1n 5 c\u00f3 th\u1ec3 l\u00ean \u0111\u1ebfn 6.6 t\u1ea5n CO2 m\u1ed9t n\u0103m, t\u01b0\u01a1ng \u0111\u01b0\u01a1ng ph\u00e1t th\u1ea3i c\u1ee7a m\u1ed9t chuy\u1ebfn bay kh\u1ee9 h\u1ed3i H\u00e0 N\u1ed9i \u2013 New York, v\u00e0\u00a0t\u1ed5ng ph\u00e1t th\u1ea3i h\u1eb1ng n\u0103m t\u1eeb vi\u1ec7c \u0111i\u1ec1u tr\u1ecb c\u00e1c b\u1ec7nh nh\u00e2n CKD giai \u0111o\u1ea1n cu\u1ed1i t\u1ea1i Vi\u1ec7t Nam hi\u1ec7n nay \u01b0\u1edbc kho\u1ea3ng 0,5 tri\u1ec7u t\u1ea5n CO<\/span><span style=\"font-weight: 400;\">\u2082 &#8211; <\/span><span style=\"font-weight: 400;\">t\u01b0\u01a1ng \u0111\u01b0\u01a1ng 1 m\u00e1y bay \u0111\u1ea7y kh\u00e1ch bay v\u00f2ng quanh tr\u00e1i \u0111\u1ea5t 833 l\u1ea7n [9]. C\u00e1c ph\u00e2n t\u00edch n\u00e0y cho th\u1ea5y n\u1ebfu\u00a0l\u00e0m ch\u1eadm ti\u1ebfn tri\u1ec3n CKD, t\u0103ng c\u01b0\u1eddng\u00a0ph\u00e1t hi\u1ec7n s\u1edbm v\u00e0 qu\u1ea3n l\u00fd t\u1ea1i c\u1ed9ng \u0111\u1ed3ng, \u0111\u1ed3ng th\u1eddi\u00a0t\u1ed1i \u01b0u m\u00f4 h\u00ecnh \u0111i\u1ec1u tr\u1ecb thay th\u1ebf th\u1eadn (\u01b0u ti\u00ean l\u1ef1a ch\u1ecdn t\u1ea1i nh\u00e0 khi ph\u00f9 h\u1ee3p, gi\u1ea3m di chuy\u1ec3n v\u00e0 s\u1eed d\u1ee5ng t\u00e0i nguy\u00ean), ch\u00fang ta kh\u00f4ng ch\u1ec9 gi\u1ea3m chi ph\u00ed y t\u1ebf tr\u1ef1c ti\u1ebfp cho ng\u01b0\u1eddi b\u1ec7nh v\u00e0 h\u1ec7 th\u1ed1ng, m\u00e0 c\u00f2n\u00a0gi\u1ea3m \u0111\u00e1ng k\u1ec3 d\u1ea5u ch\u00e2n carbon c\u1ee7a ng\u00e0nh y t\u1ebf.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u0110\u00e2y l\u00e0 b\u1eb1ng ch\u1ee9ng \u0111\u1ea7u ti\u00ean t\u1ea1i Vi\u1ec7t nam cho th\u1ea5y <\/span><b>m\u1ed1i li\u00ean k\u1ebft r\u00f5 r\u00e0ng gi\u1eefa m\u1ee5c ti\u00eau s\u1ee9c kh\u1ecfe v\u1edbi m\u1ee5c ti\u00eau ph\u00e1t tri\u1ec3n b\u1ec1n v\u1eefng b\u1ec1n v\u1eefng<\/b><span style=\"font-weight: 400;\">, b\u1ed5 sung kh\u00eda c\u1ea1nh m\u00f4i tr\u01b0\u1eddng \u2013 kinh t\u1ebf x\u00e3 h\u1ed9i v\u00e0o ho\u1ea1ch \u0111\u1ecbnh ch\u00ednh s\u00e1ch CKD [9,3], nh\u1eb1m g\u00f3p ph\u1ea7n gi\u1ea3m thi\u1ec3u t\u00e1c \u0111\u1ed9ng l\u00ean m\u00f4i tr\u01b0\u1eddng c\u1ee7a h\u1ec7 th\u1ed1ng y t\u1ebf, h\u01b0\u1edbng t\u1edbi cam k\u1ebft \u0111\u1ea1t Net Zero v\u00e0o n\u0103m 2050 c\u1ee7a Ch\u00ednh ph\u1ee7.<\/span><\/p>\n<p><b>Nguy\u00ean nh\u00e2n v\u00e0 y\u1ebfu t\u1ed1 nguy c\u01a1: \u0111\u1ecbnh h\u01b0\u1edbng ph\u00f2ng ng\u1eeba theo nh\u00f3m nguy c\u01a1 cao<\/b><\/p>\n<p><span style=\"font-weight: 400;\">CKD kh\u1edfi ngu\u1ed3n t\u1eeb t\u1ed5n th\u01b0\u01a1ng t\u1ea1i m\u1ea1ch th\u1eadn, c\u1ea7u th\u1eadn, \u1ed1ng \u2013 m\u00f4 k\u1ebd v\u00e0 \u0111\u01b0\u1eddng ti\u1ebft ni\u1ec7u. C\u00e1c y\u1ebfu t\u1ed1 nguy c\u01a1 ph\u1ed5 bi\u1ebfn g\u1ed3m \u0111\u00e1i th\u00e1o \u0111\u01b0\u1eddng, t\u0103ng huy\u1ebft \u00e1p, tu\u1ed5i cao, b\u1ec7nh tim m\u1ea1ch, ti\u1ec1n s\u1eed t\u1ed5n th\u01b0\u01a1ng th\u1eadn c\u1ea5p, ti\u1ec1n s\u1eed gia \u0111\u00ecnh m\u1eafc b\u1ec7nh th\u1eadn m\u1ea1n, h\u00fat thu\u1ed1c, b\u00e9o ph\u00ec, s\u1eed d\u1ee5ng k\u00e9o d\u00e0i thu\u1ed1c ch\u1ed1ng vi\u00eam kh\u00f4ng steroid (NSAIDs) v\u00e0 c\u00e1c thu\u1ed1c \u0111\u1ed9c th\u1eadn&#8230; [2]<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u1ede b\u1ec7nh nh\u00e2n ung th\u01b0, t\u1ed5n th\u01b0\u01a1ng th\u1eadn c\u1ea5p v\u00e0 CKD th\u01b0\u1eddng g\u1eb7p v\u1edbi c\u01a1 ch\u1ebf \u0111a y\u1ebfu t\u1ed1 (th\u00e2m nhi\u1ec5m, h\u1ed9i ch\u1ee9ng ti\u00eau kh\u1ed1i u, t\u1eafc ngh\u1ebdn, nhi\u1ec5m tr\u00f9ng, \u0111\u1ed9c t\u00ednh t\u1eeb h\u00f3a tr\u1ecb\/x\u1ea1 tr\u1ecb v\u00e0 thu\u1ed1c \u0111i\u1ec1u tr\u1ecb \u0111\u00edch nh\u01b0 platinum, anti VEGF\u2026), \u0111\u00f2i h\u1ecfi ph\u1ed1i h\u1ee3p \u0111a chuy\u00ean khoa v\u00e0 s\u1ef1 tham gia s\u1edbm c\u1ee7a b\u00e1c s\u0129 th\u1eadn.<\/span><\/p>\n<p><b>Kho\u1ea3ng tr\u1ed1ng trong t\u1ea7m so\u00e1t v\u00e0 qu\u1ea3n l\u00fd: \u01b0u ti\u00ean n\u00e2ng cao n\u0103ng l\u1ef1c tuy\u1ebfn c\u01a1 s\u1edf<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Kh\u1ea3o s\u00e1t th\u1ef1c tr\u1ea1ng ch\u1ea9n \u0111o\u00e1n, theo d\u00f5i v\u00e0 t\u1ea7m so\u00e1t CKD (H\u1ed9i ngh\u1ecb Ti\u1ebft ni\u1ec7u \u2013 Th\u1eadn h\u1ecdc to\u00e0n qu\u1ed1c, 8\/2025) cho th\u1ea5y t\u1ef7 l\u1ec7 b\u1ec7nh nh\u00e2n nguy c\u01a1 cao \u0111\u01b0\u1ee3c l\u00e0m \u0111\u1ee7 x\u00e9t nghi\u1ec7m ch\u1ea9n \u0111o\u00e1n c\u00f2n th\u1ea5p; h\u01a1n m\u1ed9t ph\u1ea7n ba c\u01a1 s\u1edf ch\u1ec9 th\u1ef1c hi\u1ec7n \u0111\u1ea7y \u0111\u1ee7 cho d\u01b0\u1edbi 25% b\u1ec7nh nh\u00e2n. T\u1ea7n su\u1ea5t x\u00e9t nghi\u1ec7m ch\u1ee7 y\u1ebfu 1 l\u1ea7n\/n\u0103m; m\u1ed9t b\u1ed9 ph\u1eadn th\u1ef1c hi\u1ec7n kh\u00f4ng \u0111\u1ecbnh k\u1ef3, d\u1eabn t\u1edbi b\u1ecf s\u00f3t ch\u1ea9n \u0111o\u00e1n. [10]<\/span><\/p>\n<p><span style=\"font-weight: 400;\">H\u01b0\u1edbng d\u1eabn ch\u1ea9n \u0111o\u00e1n v\u00e0 \u0111i\u1ec1u tr\u1ecb CKD (B\u1ed9 Y t\u1ebf, 2024) nh\u1ea5n m\u1ea1nh ph\u00e1t hi\u1ec7n s\u1edbm t\u1ea1i tuy\u1ebfn c\u01a1 s\u1edf v\u00e0 s\u1eed d\u1ee5ng s\u1edbm c\u00e1c nh\u00f3m thu\u1ed1c c\u00f3 ch\u1ee9ng c\u1ee9 b\u1ea3o v\u1ec7 th\u1eadn \u0111\u1ec3 l\u00e0m ch\u1eadm ti\u1ebfn tri\u1ec3n, gi\u1ea3m bi\u1ebfn c\u1ed1 tim m\u1ea1ch \u2013 th\u1eadn.
